Making an Appointment

On your first visit, we will begin a permanent record. We will ask you to complete a registration form and a medical history. Please bring your insurance card and try to arrive thirty minutes early for your first visit.

Please ask your referring physician to forward pertinent medical records to us at fax 972 981 7636. This is particularly important for new patients as it may provide important information regarding your referral, expedite your care, and avoid repeating tests unnecessarily.

In order to save you time, you are welcome to download Patient Forms, print it, fill it in (as much information you can) and bring it with you. This will save you some time and expedite the signing-in process.

Please bring all your medications or a current updated list of your medications to all visits to keep your records accurate, to avoid confusion, and to help prevent improper dosages or drug interactions. In addition, diabetics should bring their glucometer and blood sugar (glucose) records to all visits.

In order to keep our patients on schedule, we create appointments with adequate time for proper evaluation and treatment. If you believe you will not be able to make to your appointment, please call our office in advance and re-schedule the appointment. If you cannot keep your appointment, please call at least 24 hours in advance for a return visit and 48 hours in advance for a new visit so that we may accommodate another patient. Also, if you are running late, we appreciate being informed so that we can adjust our schedule accordingly.
